Canon has recently released a new firmware update for the EOS 5D Mark II:. Changes: Firmware Version 2.1.1 incorporates the following fixes and improvements. 1. Fixes a phenomenon where shooting stops after capturing one image when in continuous shooting or the Auto Exposure Bracketing (AEB) continuous shooting setting.
Make sure the file is named correctly and not something like file.ext1.ext2 with an extra incorrect extension. Use the file info (on a mac) or file properties on a PC.

You're right. After trying out new camera on both pc and mac, we tried a friends 5d mkiii with the extension cable and it's working. So, after spending several hours on this, I believe it's the camera itself.

I tested this with one of my 600EX-RT units put the lens deliberately well out of focus, then let the camera auto-focus and repeated this about 20 times. It's pretty fast. I swapped the 600 for a 580EX II and repeated the test.
